Do you still have some of the ccp?
Advise the CPSC they may release your
name to the manufacturer,
and public. Include any physician's
report, testing (include any
blood test that list chemicals in your
blood from the ccp).
You may black out your physician's name
if needed.
***ASK YOUR PHYSICIAN TO SUBMIT THE FOLLOWING:
HEALTH CARE PHYSICIAN'S CONSUMER PRODUCT INCIDENT
REPORT.
Must contain:
Health Care Provider's complete name, address, city and state
Physician's telephone number
Victim or injured person's name, address, city and state
Victim's age and sex
Date of injury or incident
Describe the incident or injury and diagnosis. Be specific
Describe the product carbonless Copy Paper
Product name and manufacturer
Place where manufactured if known, city, state
If ccp used at work, name of employer, address, city and state
Where was product purchased or used?
Is the product available for examination
If yes, where is it located?
**ASK YOUR PHYSICIAN TO REQUEST HIS LETTER BE PUT IN THE
PUBLIC FILE
*SEND A COPY OF YOUR COMPLAINT TO BOTH

ASK that your letter
